This form is to be completed and submitted with your Associations PARTICIPATION SUMMARY for attending the WCJJO World Jujitsu Championships 7th to 12th August 2019 in Orlando Florida USA

… ‘PART A’ refers to the person nominated to represent your Association and their contact details … ‘PART B’ advises the number of MALE competitors and ‘PART C’ the number of FEMALE competitors in both the Sport Ju-Jitsu and the Grappling Divisions … ‘PART D’ advises if you will have a Demonstration Team … ‘PART E’ advises if you have experienced people who can assist with officiating and other duties … ‘PART F’ is your TOTAL number of people attending, this should include everyone: competitors, officials, and support people … Question 19 and 20 (see note) … and finally the name of the person submitting this information and date.
